Process for Pharmacist Licensure in Indiana*
(1) Attend an ACPE-accredited
(American Council on Pharmacy Education) School:
- Near completion of Doctor of Pharmacy
(Pharm.D.) curriculum, student should complete Indiana State Board of
Pharmacy's application (www.pla.in.gov)
- Submit $100.00 application fee to the
Indiana State Board of Pharmacy
- Submit one passport quality photograph
- Submit "Certificate of Completion"
from pharmacy school
- Contact the NABP at
www.nabp.net or 847-391-4406 to pay the application fees for the
NAPLEX ($465) and MPJE ($185)
- Pass the NAPLEX and MPJE with a
minimum score of 75%
(2) Attend School Not Accredited by
ACPE:
- Complete Indiana State Board of
Pharmacy application (www.pla.in.gov)
- Submit $100.00 application fee to the
Indiana State Board of Pharmacy
- Submit one passport quality photograph
- Submit "Certificate of Completion"
from Pharmacy School
- Submit verification of having 1,500
hours of internship (must come from the State Board of Pharmacy where
the hours were completed)
- Contact the NABP at
www.nabp.net
or 847-391-4406 to pay the application fees for the NAPLEX ($465) and
MPJE ($185)
- Pass the NAPLEX and MPJE with a
minimum score of 75%
(3) Foreign Pharmacy Graduates:
- Complete Indiana State Board of
Pharmacy application (www.pla.in.gov)
- Submit $100.00 application fee to the
Indiana State Board of Pharmacy
- Submit one passport quality photograph
- Submit a Notarized copy of FPGEC
(Foreign Pharmacy Graduate Examination Committee) Certification
- Contact the NABP at
www.nabp.net or 847-391-4406 to pay the application fees for the
NAPLEX ($465) and MPJE ($185)
- Pass the NAPLEX and MPJE with minimum
score of 75%
- Complete and
submit verification of 1,500 hours of internship (may be completed at
any time during the application process, however all hours must be
submitted prior to the license being issued)
