2017 APhA the Pharmacist and Patient Centered Diabetes Care Certificate Training Program is an intensive educational experience designed to equip pharmacists with the knowledge, skills and confidence needed to supply effective, evidence basd diabetes care. Five self-study modules provide comprehensice instruction in current diabetes concepts and standards of care. The live seminar incorporates case studies and hands-on skills training focused on situations likely to be encountered. - as well as the services most needed - in community and ambulatory practice settings.
Participants will gain experience evaluating and adjustng drug therapy regiments for patients with Type I and Type II Diabetes, counseling patients about lifestyle interventions, analyzing and interpreting self monitoring of blood glucose results and accessing overall health status of patients to identify needed monitoring and interventions.
Course material and exams will be accessed online - In order to participate in this activity, Registrants must have access to a computer with minimum system requirements: Internet connectivity with current version of internet browsers, such as Chrome, Firefox, Safari, or Internet Explorer (V9 and above); Adobe Acrobat Reader, Flash Player 8 or higher, Windows 95, Pentium 3 or equivalent processor, 64 MB of free memory (not 64 MB total), and Audio: Sound card and speakers or earphones.
