Program: Medication Safety Awards
Award Recognizing
an Individual Practitioner
Overview
The
IPA/MSP medication safety award is presented to individual healthcare
practitioners that demonstrate a dynamic and enthusiastic commitment to
medication-use safety through innovative and creative projects,
programs, educational efforts, and/or research. Winning individuals have
implemented medication safety strategies into their daily clinical
practice.
Eligibility
To be eligible for the award, healthcare practitioners must
be licensed or certified in the State of Indiana (e.g. registered
pharmacist, registered nurse, physician, pharmacy technician, licensed
practical nurse, etc.). Also, healthcare practitioners applying for
this award must have their license or certification in good standing
with the State of Indiana.
Selection
Criteria
Nominations for the award will be evaluated on three primary
criteria:

Commitment to the safe use of medications over time through
innovative and creative practice techniques
-
A willingness to share learning regarding medication safety
through presentation, written articles, participation in collaborative
groups and other methods (newsletters, intranet, webinars, etc)
- Demonstrated
effectiveness in their current practice with regard to medication
safety
Selection
Process
Nominations will be solicited annually by the Indiana
Pharmacists Alliance. The nominator
will be asked to complete an essay describing how the
individual
have
met the award criteria (maximum of 1500 words). The essay will be
evaluated, through a blinded process, by a review committee selected by
the Indiana Pharmacist Alliance and the Medication Safety Partnership
for Indiana from among its members. The essay must be completed prior
to an established deadline to be evaluated by the committee.
Award
The award will be accompanied by a
recognition plaque and a
$250
check to the recipient.

Award Recognizing an Organization
Overview
The
IPA/MSP medication safety award is presented to a healthcare
organization that demonstrates a dynamic and enthusiastic commitment to
medication-use safety through innovative and creative projects,
programs, educational efforts, and/or research. Winning organizations
have implemented medication safety strategies into their organization,
including a culture of safety among the staff.
Eligibility
To be eligible for the award, a healthcare
organization must apply for the award or be nominated by a person.
The term healthcare organization can include a wide variety of entities
including organizations such as: pharmacies, insurance companies,
hospitals, long term care facilities, professional organizations,
patient safety organizations, etc.
Selection Criteria
Nominations for the award will be evaluated
on
four
primary criteria:

Commitment to the safe use of medications
over time through innovative and creative practice techniques

A willingness to share learning regarding
medication safety through presentation, written articles,
participation in collaborative groups and other methods (newsletters,
intranet, webinars, etc)

Demonstrated effectiveness in their current
practice with regard to medication safety

Demonstrated commitment to the concept of a “Culture of Safety”
throughout the organization
